Explaining Breast Self Exam (BSE) to a Spanish Speaking Patient  

You: Le he explicado el procedimiento para hacerse el examen de los senos.
I’ve explained to you the procedure to do a breast self-exam.
You: Clara, ¿tiene alguna pregunta sobre lo que le he explicado o está bien todo?

You: ¿No tiene ninguna otra pregunta?

You: Ahora lo que vamos a hacer es que se va a cambiar, le voy a dar esta bata corta para que se cambie. Quitese solamente lo de arriba, su brassiere y su blusa, y se va a poner esta bata con lo abierto para en frente, ¿OK?

Pacient: Todo esto me pone muy nerviosa, entonces como que no me acuerdo de lo que pasó. Cuando trato de hacerlo hasta siento que a lo mejor lo estoy haciendo mal y no sé. No sé que es lo que está mal o que es lo que está bien, porque me toco y al tocarme no sé si es normal o si no es normal. No estoy acustumbrada a tocarme.

This makes me very nervous, and then it seems like I don’t remember what happened. When I try to do it, I have the feeling that I am doing it wrong, and I don’t know. You: Bueno, yo le voy a enseñar tres pasos que nosotros consideramos necesarios para hacer un buen examen. Hay tres posiciones. La primera posición es parada o sentada mirándose en un espejo. Well, I’ll teach you three steps that we consider necessary for doing a good breast self-exam. There are three positions. The first one is standing or sitting in front of a mirror. The second position is touching yourself while you are in the shower; and the third one is lying down. In all the positions you are looking for changes in your breasts.
Mirror Section Conversation
You: Pero lo primero que va a hacer es mirarse en un espejo. Quiere mirarse con las manos relajadas al lado de sus piernas, y lo que está haciendo es solamente viéndose y lo que quiere buscar es-- que no quiere encontrar, siempre les digo a las pacientes, busquen pero esperemos no encontrar nada-- es enrojecimiento en la piel, hoyuelos, sequedad alrededor del pezón, venas muy, muy inflamadas. Si están superinflamadas ese es un cambio. No quiere ver eso en sus senos. Otra vez, sequedad, quiere hacer el examen poniendo sus manos en la cadera, empujando hacia abajo con los brazos enfrente. Lo mismo está haciendo, está buscando cambios.
The first thing you are going to do is look at yourself in the mirror. You want to look at yourself with your arms relaxed at your sides, and you are only looking at yourself, and what you are looking for is –that you don’t what to find, I always tell my patients, look for but hopefully we won’t find any— is redness of the skin, dimpling, dryness around the nipples, very very swollen veins. If they are too swollen, that means there is a change. You don’t want to see that in your breasts. Once again, dryness, you want to do the exam with your hands pressing down on your hips. You are doing the same, looking for any changes.
You: Ok, en esta parte, cuando hace esto, esto le ayuda a verse la forma de los senos. Okay, in this step, when you do that, this helps you see the shape of your breasts. Leaning, Clavicle Conversation
You: Cuando termine con esta posición tiene que agacharse, quiere dejar que sus senos se quiten de su piel, que se despeguen de su piel y quiere asegurarse que sus pezones apunten en la misma dirección, que no esté uno para un lado y el otro para otro. Eso no es normal.
When you are done with that position, you have to bend forward a let your breasts stay away from your skin, and you have to make sure the nipples are facing in the same direction, not one facing one way, and the other one another. That is abnormal.

You: Quiere asegurarse de no sentir nada que se sienta como un chícharo o un frijol. Después de esa parte quiere recorrer su seno desde la clavícula hasta el pezón. You want to make sure that you don’t feel anything like a pea or a bean. After that, you want to sweep your fingers from your collarbone down to your nipple. Once again, there is breast tissue all over this area, not only in the breasts, so it is important to examine this area. Women sometimes think that the breast area is only the breast.

Fingertips, cystic tissue
You: Ahora vamos a examinar su seno. OK, ésta es la segunda parte. Usted quiere examinar su seno como si estuviera agarrando una concha, recuerde utilizar la mano opuesta para examinar. Lo más importante es examinar toda el área del seno usando las yemas de los dedos, sin alzar los dedos. Si Ud. levanta los dedos puede ser que haya habido algo ahí y ya no lo encuentre.
Now we are going to examine your breast. Okay, this is the second part. You want to examine your breast as if you were holding a shell; remember to use the opposite hand. The most important thing is to examine all the breast area using your fingertips, without lifting your fingers. If you lift your fingers you might miss something.

That is not a problem.
You: No, no es problema, el problema es cuando sentimos algo muy duro que no se mueve. OK, cuando tenemos algo que se mueve es menos preocupante, pero cuando Ud. examine sus senos y encuentre un bulto en un solo lado, quiere examinarse el otro lado para ver si se siente allí. Lo que más nos preocupa es cuando hay un bulto en un solo lado y no se mueve. OK, entonces lo mas importante es examinar toda el área del seno..

When you reach the nipple, squeeze it gently and pull it forward. You want to make sure it springs back into place. We are going to give you a brochure with pictures of the different positions you use for the exam, and we’ll also give you a card that you can place in the closet or in the shower or bathroom.
